Comparing the backpack part of the top image to the lump in the bottom image, I think that fits perfectly. It also ties in with him missing his physical this year, with falling off the Segway, and with him "choking on a pretzel" and then passing out. Perhaps our Commander-in-Chief has had some cardiac trouble?

The first debate when it looked like he had a pack of cigs on his back led to a lot of speculation that he was wearing a "cheat" wire. I believe that that turned out to be a backup mic-box. Each candidate had two, the primary, and the backup they wore under their shirts.
The picture above, however, looks suspiciously like that vest. Or, it could be a bullet-proof vest that the SS (heh) asked him to wear.

You guys obviously overlooked the obvious possibility that Bush is some kind of lizard monster and that the bony bumps on his back are starting to poke through because his human suit was poorly made.
