I would like to add a comments system to my website.
I want something simple, easy, no-frills, low-maintenance and free.
Any advice on what I should use?

I used to use this on iwn back when I first started it. It's fairly basic (flatfile db), but easy to setup and use if you're comfortable with editing php files.
Failing that, Yahoo or Clusty.
