I am a huge Horror movie buff. I love anything that makes me scared, and to be honest, hardly anything in the last decade has done so. I long for that movie that makes me feel like something is gonna grab my feet at the edge of the bed when I sleep. That maybe I'll see something moving in the mirror when I glance quickly in that direction.
The last film I watched that made me literally lose sleep was the extended version of the Exorcist. The scariest moment in film to me EVER was the spiderwalk down the stairs. Fuck, I almost shit myself at the theatre when I saw that.

I think the scariest film moment ever was when I took a lady to see Titanic and suddenly realized that I was going to have to sit through the whole thing.

I don't like horror movies that much, and they usually don't scare me. There were two movies that scared the piss out of me when I was a kid, but I can't point to any specific scenes as I haven't seen them in 30 years.

The first was, I think, named "Ben," about some evil rats that get together and do some bad things I don't remember. I laid awake at night for a long time scared of rats.

The second was "Food of the Gods," about insects exposed to radioactive waste and growing way too large. It would probably be really fucking dumb if I saw it today but I didn't think it was when I had to ride my bike home at night all by my self. Freaked me out man.

Though more disturbing than scary, the image of the slug in the lettuce from Slugs stayed with me a for years.

As for scary, the most vivid jump I can remember was the kitchen scene from Jacob's latter where Jezzie is trying to get Jacob's attention. She taps on his head and finally growls, "Anybody in there?" with those terrifying totally black eyes. Amazing how something so seemingly mundane can give a fright...

Alien, in the theater in '79 I think, on opening night. I was 14 and expecting sci-fi fun, post-Star Wars. Yeah. Not so much. The fact that the rest of the audience freaked out too just made me freak out more. Took me a while to stop twitching at shadows.

Roy Schnieder sticking his head over the side of the boat in Jaws, and almost getting it chomped off.

The video sequence from the Ring creeped me out too.

Arachnophobia, the whole fuking movie (I have a freakishly girlish phobia of spiders).

The only thing ill remember:

The sex scene in "Lost Highway"

Bill Pulman fucks his wife, the sounds in the background are dark and silent, like those in nearly all horror movies which were designed to create a unpleasent "something is gonna to happen" atmosphere.
Then, he quits and says "Ive had a dream, i laid in bet, like yet. You were beside me, but it wasnt you, ill looked in your face, but it wasnt yours".
Then he turns the light on, looks to his wife and sees the face of another man.
I can clearly remember his face, but that wasnt decisive point, the whole scene were scary for me.

when i was in 5th grade (30 yrs ago) i checked out a book from the biblio called 'True Horror Stories" it had the mothman story, but the mothman attacked cars at night, and they found a cop car with the top peeled off, and the cop gone.

I was so disapointed by the movie

Prince of Darkness (1987) In a scene near the end a hand reaches through a mirror from the worng side, it is big and black and evil nasty. It is the devils hand and it still scares the shizen out of me.

And although it wasn't really a horror flick, the scene in Misery where Cathy Bates breaks James Caan's ankles with a sledgehammer. First time I ever screamed in a movie theater.

The final scene of Carrie when her hand burst up from the drave and grabs the girl who is putting down flowers. Most shocking moment in horror flicks. I saw this in the theatre when I was about 10. Brutal nightmares for a while thereafter.

Misery was one book adaptation that did not disappoint.

28 days later was great. I thoroughly enjoyed The Ring and Jeepers Creepers. But I can't remember a movie actually scaring me beyond a pleasant suspenseful stomach drop since I saw Exorcist.

But Saw came pretty damn close. Really close.

The whole movie built tension. I was genuinely repulsed in the scene with the pocket knife/lockjaw. On reflection it wasn’t a good idea to see the movie with a hangover while eating pizza and chicken strips but I still enjoyed it. I like get-under-your-skin horror as opposed to the cheap startle-scare tactics.

Another suggestion of Mothman Prophecies here.

Oh yeah, and 'Session 9'...while a little campy, the first 'What are you doing here?' lobotomy scene was pretty fucked up.

Another was a 1985 anime called "Tenshi no Tamago" (Angel Egg) by Mamoru Oshi, while not really scarey, once you sit through the first fifteen minutes or so, things get surreal and eerie.
